WAIVER & Release of Liability

Acknowledgment of Risk

I certify that my child has no known medical or other conditions that would interfere with his/her safe participation in GSS soccer activities. I understand and accept that the practice of soccer, both indoor and outdoor, involves inherent risks of physical injury.

Release of Liability

I, individually and on behalf of my child, hereby release, discharge, and hold harmless Danny McGhee (owner of Global Freestyle, LLC), along with its officers, directors, agents, representatives, employees, successors, assignees, and licensees (collectively, the “Released Parties”), from any and all claims, actions, damages, losses, liabilities, costs, or expenses of any kind whatsoever.

This includes, but is not limited to, any claims of negligence, arising out of or in connection with my child’s participation in any GSS program or activity.

I agree that I will not bring or participate in any legal action or claim against the Released Parties related to my child’s participation, under any legal theory whatsoever (including personal injury, negligence, rights of privacy and publicity, or defamation).

Program Rules & Supervision

I acknowledge that Danny McGhee reserves the right to terminate the enrollment of any child if, in his sole discretion, participation is deemed not in the best interest of the child or the program.
I agree to supervise or designate a responsible adult to supervise my child immediately before and after his/her soccer activities.
I understand that GSS staff are responsible only during the scheduled program time.

Medical Emergencies

In the event of a medical emergency, I authorize GSS staff to obtain necessary medical treatment for my child and agree to assume full financial responsibility for such treatment.

Photo & Video Release

I grant GSS permission to use photos or videos of my child participating in programs for promotional, educational, and marketing purposes, without compensation.

Acknowledgment

By completing my child’s registration, I certify that I have read, understood, and agreed to this Waiver & Release of Liability. I acknowledge that checking the box at checkout serves as my electronic signature of consent.
